Thesis Advising Document

Please complete and attach this form to the Thesis Plan and Enrollment Form, or with the Incomplete Contract each time—but only if—it is needed.

It is needed with the Thesis Plan and Enrollment Form for the following reasons:

• to address students' knowledge of the IRB process;
• to confirm the number of thesis development credits being enrolled for;
• and to confirm the completion of or address the progress of the QP and TP.

It is needed with the Incomplete Contract for the following reason:

• to verify that the student meets the criteria for 75% completion of the thesis.

ProQuest Publishing Instructions
Required of all students entering fall 2007 or later.

Thesis Binding
This is primarily for students who want colored images in their thesis.
